Required Documentation

  • A valid State ID, Driver's License, or Passport.
  • Ohio Revised Code and/or reason for background check.

  • Documentation of employment/volunteer placement requested. Includes volunteers with a letter from the  school district.

  • The contact name and address of where results need to be mailed at time of appointment.
